I certify that my dog is in good health and is up to date on all vaccinations required by Ontario law and by Wishing Creek Farm, including DA2PP, Rabies, Bordetella, Leptospirosis & Lyme.

I understand that proof of vaccination is required prior to attending Daycare or Boarding.*

I understand that:

There are inherent risks in dog park use, including exposure to other animals, insects, and plants, and the potential for injury, illness, or stress.

Wishing Creek Farm will take reasonable precautions for safety, but cannot guarantee my dog will not become ill or injured.

I am responsible for any costs related to veterinary care, illness, or injury that occurs while my dog is at Wishing Creek Farm. *

If my dog becomes ill or injured while in care, I authorize Wishing Creek Farm to seek veterinary care at their discretion.

I agree to pay all associated veterinary costs directly to the clinic.

If my preferred vet is not available, I authorize Wishing Creek Farm to seek veterinary treatment or assistance with:

I confirm that I will abide by the rules of Wishing Creek Farm, including:

Not arriving early or staying late for my appointment.

Not getting out of my vehicle if it is not safe to do so (ie. if another guest has not left yet).

Not driving faster than 20 km/hr anywhere on the property.

Not consuming drugs or alcohol while on the premises.

Staying within my designated rental area.*

I understand that Wishing Creek Farm is not responsible for loss or damage to any personal items (e.g., leashes, toys, etc.) brought with my dog.*

By signing below, I release and hold harmless Wishing Creek Farm, its owners, employees, and agents from any and all liability, claims, or damages resulting from injury, illness, or death of my dog, except in cases of proven negligence or willful misconduct.*
